Xylitol: A Sweetener With a Complicated Story
If you have ever chewed sugar-free gum, used certain toothpastes, or bought a “sugar-free” or “keto-friendly” product, you may have come across xylitol.
Xylitol is a type of sugar alcohol, also called a polyol. It tastes similar to sugar but contains fewer calories and has a much smaller effect on blood glucose. It is therefore often promoted as a healthier alternative to regular sugar.
But there is more to the story.
Xylitol has some potential advantages, particularly for oral health. At the same time, it can cause digestive symptoms when consumed in larger amounts. More recently, research has raised questions about whether high exposure to xylitol could affect blood clotting and cardiovascular health.
So, is xylitol healthy?
The answer is not simply yes or no. How much you consume, why you are using it, and what product it is in all matter.
Let's take a closer look.
What Is Xylitol?
Xylitol is a sugar alcohol that occurs naturally in small amounts in some fruits and vegetables. It can also be produced commercially from plant materials.
Despite its name, xylitol is not the same as alcoholic beverages. The term sugar alcohol refers to its chemical structure.
Xylitol tastes about as sweet as ordinary table sugar, but it provides fewer calories because the body does not completely absorb and metabolize it.
You may find xylitol in:
Sugar-free chewing gum
Sugar-free mints and candies
Some baked goods
Sugar-free desserts
Toothpaste
Mouthwash
Some medicines and supplements
The ingredient may also appear on labels as E967.
Xylitol vs Sugar: What's the Difference?
The biggest reason people choose xylitol is that it provides sweetness without behaving exactly like ordinary sugar.
Regular table sugar is rapidly broken down into glucose and fructose. Xylitol is absorbed differently and has a much smaller effect on blood glucose.
This makes xylitol attractive to people trying to reduce their sugar intake.
But there is an important distinction:
Replacing sugar with xylitol may reduce sugar consumption, but that does not automatically make a food healthy.
A cookie, candy, or dessert can still be highly processed even if it contains xylitol instead of sugar.
The overall food matters more than the sweetener alone.
Does Xylitol Raise Blood Sugar?
One potential advantage of xylitol is its relatively small effect on blood glucose and insulin compared with ordinary sugar.
Because xylitol is absorbed and metabolized differently from glucose, it does not cause the same rapid rise in blood sugar.
This is one reason it is commonly used in products marketed as:
Sugar-free
Low-sugar
Diabetic-friendly
Keto-friendly
However, “sugar-free” does not mean unlimited.
Xylitol still contains calories and carbohydrates, and products containing xylitol may contain other ingredients that affect blood glucose.
If you have diabetes, it is better to look at the whole nutrition label rather than focusing on one ingredient.
Is Xylitol Good for Your Teeth?
This is probably the area where xylitol has received the most attention.
Unlike ordinary sugar, xylitol is not readily fermented by the bacteria in dental plaque in the same way. Chewing xylitol gum can also stimulate saliva production.
Saliva helps neutralize acids in the mouth and supports the natural repair of tooth enamel.
Some research has found that xylitol can reduce levels of Streptococcus mutans, bacteria associated with dental caries.
However, the evidence is not as clear-cut as some advertisements suggest.
A 2024 systematic review of nine human clinical trials concluded that the evidence was not sufficient to confirm that xylitol prevents dental caries, partly because studies used different doses, products, and treatment schedules.
Earlier reviews have reported possible benefits, particularly with xylitol chewing gum, but the quality and consistency of the evidence have varied.
So it is better to think of xylitol as a potentially useful addition to good oral hygiene, rather than a replacement for brushing with fluoride toothpaste, flossing, or regular dental care.
A practical tip
If you enjoy chewing gum, choosing a sugar-free gum containing xylitol may be preferable to chewing gum sweetened with ordinary sugar.
The chewing itself also increases saliva flow.
Xylitol and Gut Health
This is where things can get complicated.
Xylitol is only partially absorbed in the small intestine. Some of it reaches the large intestine, where intestinal bacteria can ferment it.
This can produce gas and draw water into the bowel.
For some people, especially when larger amounts are consumed, this can result in:
Bloating
Gas
Abdominal discomfort
Loose stools
Diarrhea
Xylitol is classified as a polyol, one of the carbohydrate groups included in the FODMAP category. Polyols can be poorly absorbed and may trigger digestive symptoms in people who are sensitive to them, including some people with IBS.
This does not mean that everyone needs to avoid xylitol.
Tolerance varies from person to person.
A small amount in chewing gum may cause no problems, while consuming large quantities of xylitol-sweetened sweets or desserts may cause significant digestive symptoms.
Can Xylitol Cause Diarrhea?
Yes.
This is one of the best-established side effects of consuming large amounts of xylitol.
Because unabsorbed xylitol can draw water into the intestine, larger amounts may have a laxative effect.
You may notice:
Loose stools
Urgency
Diarrhea
Bloating
Abdominal cramps
Gas
This is why some products containing sugar alcohols carry a warning that excessive consumption may have a laxative effect.
If you are new to xylitol, start with small amounts rather than suddenly consuming large quantities.
The New Concern: Xylitol and Heart Health
This is the part of the xylitol story that deserves particular attention.
In 2024, researchers reported that higher blood levels of xylitol were associated with an increased risk of major cardiovascular events, including heart attack and stroke. Laboratory and human studies also found that xylitol increased platelet reactivity and promoted the formation of blood clots.
The researchers also gave healthy volunteers a drink containing 30 grams of xylitol. Their blood xylitol levels rose sharply, along with several measures of platelet activity.
But the evidence has now grown.
In 2026, researchers analyzed data from 17,710 people in two large population studies. Those with the highest blood levels of xylitol had a 57% higher risk of major cardiovascular events—including death, heart attack, or stroke—over six years compared with those with the lowest levels. In another group followed for up to 30 years, the risk was 18% higher. These associations remained after researchers adjusted for several established cardiovascular risk factors.
Does this prove that xylitol causes heart attacks or strokes?
Not yet.
The 2026 study was observational, meaning it found an association between blood xylitol levels and cardiovascular events. It cannot by itself prove that consuming xylitol directly causes heart attacks or strokes.
There is, however, a reason for concern. The earlier research provides a possible biological explanation: xylitol may increase platelet reactivity and make blood more prone to clotting.
The researchers are calling for further studies to determine the long-term cardiovascular safety of xylitol.
So we should not say:
“Xylitol causes heart attacks.”
But we should also not dismiss the findings.
A more accurate conclusion is:
High blood levels of xylitol are associated with a higher risk of cardiovascular events, and emerging research suggests that xylitol may promote platelet activation and clot formation. More research is needed to determine whether consuming xylitol directly increases cardiovascular risk.
Until we know more, it makes sense to avoid consuming large amounts of xylitol simply because a product is labelled “sugar-free” or “healthy.” For most people, reducing the overall amount of highly sweetened and ultra-processed foods may be a better long-term strategy than replacing sugar with large amounts of alternative sweeteners.
Should You Stop Using Xylitol?
For most people, there is no need to panic.
The cardiovascular research is new and still developing. It does not mean that you need to throw away your xylitol-containing toothpaste or stop using every product that contains a small amount of xylitol.
In fact, the concern is mainly about ingested xylitol, rather than the tiny amounts you may encounter in products such as toothpaste that are not intended to be swallowed.
A sensible approach is to avoid consuming large quantities of xylitol simply because a product is labelled “sugar-free” or “keto.”
As with many ingredients, more is not necessarily better.
Who Should Be Particularly Careful With Xylitol?
You may want to limit your intake or discuss it with a healthcare professional if:
You develop diarrhea or abdominal discomfort after consuming it.
You have IBS and are sensitive to polyols.
You consume large amounts of sugar-free products every day.
You have significant cardiovascular risk factors and regularly consume large amounts of xylitol.
You are replacing many foods with heavily processed sugar-free products.
The goal should not be to find an unlimited replacement for sugar.
The bigger goal is to reduce your overall dependence on very sweet foods.

There is no single sweetener that is perfect for everyone.
Xylitol

Erythritol
Erythritol is another sugar alcohol that has also been investigated for possible cardiovascular effects. Research has raised questions about its relationship with cardiovascular events and platelet activity, although the science remains unsettled.
Stevia
Stevia-derived sweeteners are much sweeter than sugar, so only small amounts are needed. They generally have little effect on blood glucose.
Regular sugar
Sugar is familiar and safe in small amounts, but excessive intake contributes to excess calorie consumption and is associated with dental caries and metabolic health problems.
The simplest option?
For many people, the best strategy is not to replace every source of sugar with another sweetener.
It is to gradually reduce the amount of sweetness in the diet.
How Much Xylitol Should You Have?
There is no universally established “ideal” daily amount of xylitol for everyone.
Studies of dental products have used different doses, and digestive tolerance varies considerably between individuals.
If you consume xylitol, pay attention to how your body responds.
A small amount in chewing gum may be well tolerated, while consuming several servings of xylitol-sweetened sweets in one sitting may lead to diarrhea.
If a product contains a large amount of xylitol, don't assume that because it is sugar-free, you can eat it freely.
Is Xylitol Natural?
You may see xylitol described as a “natural sweetener.”
There is some truth behind this claim because xylitol occurs naturally in small quantities in certain fruits and vegetables.
However, most commercial xylitol is manufactured from plant-derived materials.
More importantly, whether a sweetener is natural does not automatically tell you whether it is healthy.
Natural and healthy are not interchangeable terms.
The Bottom Line
Xylitol is neither a miracle sweetener nor something that everyone needs to avoid.
It can be useful because it provides sweetness with less impact on blood glucose than ordinary sugar, and xylitol-containing chewing gum may have benefits for oral health. However, the evidence for preventing cavities is not completely conclusive.
Its biggest established drawback is digestive discomfort. Larger amounts can cause bloating, gas and diarrhea, particularly in people who are sensitive to polyols.
More importantly, newer research has raised concerns about a possible relationship between higher xylitol exposure, platelet reactivity and cardiovascular risk. This does not prove that xylitol causes heart attacks or strokes, but it is an important reason to avoid treating xylitol as a completely risk-free sweetener until more research is available.
For everyday eating, the healthiest approach is still remarkably simple:
Eat mostly whole or minimally processed foods, limit added sugars, and don't rely on large amounts of sweeteners to make your diet taste good.
